For Medicare patients, choosing the right hospital can save the system more than $30,000 in hip or knee replacement surgery costs. Prices range from US$14,578 for hip or knee replacement surgery at the Texas Institute of Surgery at Presbyterian Hospital to US$54,285 at Parkland Memorial Hospital. 

The data was compiled by Dexur, a company that provides applications and data to healthcare organizations to improve results. Dexur's database looked at 61 facilities in the DFW area in the 2019 calendar year. All of these facilities have performed more than 11 hip or knee replacement operations without complications. These facilities have the same code as the hospital. This data uses medical insurance claim data to calculate the amount of medical insurance expenditures for each beneficiary of hospitals in the region, and ranks them according to price. 

The data shows that most of the price difference does not come from the operation itself, but from the follow-up required after the operation.

The cost of surgery in most hospitals is between US$11,000 and US$15,000, but postoperative care costs vary widely. For the most affordable hospitals, the cost after discharge is between US$3,000 and US$5,000. These additional costs are due to rehospitalization, skilled nursing, or family health after surgery. The range of expenses after discharge is a differentiating factor of the hospital, up to US$25,000. 

In many hospitals throughout DFW, such as Texas Health Presbyterian Allen, Bellescott and White Medical Center in Carrollton, Methodist Mansfield Medical Center, and North Mountain Medical City, post-discharge expenses Significantly higher than the operation itself. 

Expenses after discharge may be an indicator of the quality of surgery, because better surgery means faster and cheaper recovery. But there may be other factors, such as medication and rehabilitation compliance or other social determinants of health, such as transportation, income, and education, which may make rehabilitation more difficult, but they do not reflect the quality of care.

Hospitals in major health systems such as Texas Health Resources, Baylor Scott & White, Methodist Health System, and Medical City Healthcare are all ranked high in terms of cost, while other hospitals are the most expensive. However, in terms of cost, Texas Health owns three of the four hospitals. There are rural, urban and suburban hospitals in the entire list. The only clear conclusion is that the safety net hospital is one of the hospitals where medical insurance pays the most for hip and knee replacements. Located in Fort Worth and Dallas Parkland, JPS has two of the most expensive surgeries in the area and two of the three most expensive hip or knee surgeries. The other mode is related to the frequency of surgery. In 2019, the 6 most expensive hospitals had operations within 30 units, and most of the most efficient hospitals had more than 150 operations. As with most activities, the frequency of task execution can improve quality and efficiency.  

Since health insurance covers most of these costs, patients have little incentive to choose more affordable facilities. Nonetheless, because price changes are closely related to expenses after discharge, a more affordable hospital usually means that the patient recovers quickly and ultimately does not return to the hospital.
